Photography & Video Waiver and Consent 
The Pancreatic Cancer Action Network will be videotaping and taking photographs at PanCAN’s Scientific Summit. By registering for or attending PanCAN’s Scientific Summit, I grant permission to the Pancreatic Cancer Action Network (and its designees and agents) to utilize my image, likeness, actions, and statements in any live or recorded audio, video, film, or photographic display or other transmission, exhibition, publication, or reproduction made of, or at, PanCAN’s Scientific Summit in any medium, whether now known or hereinafter created, or context for any purpose, including educational or promotional purposes, without further authorization or compensation.
